This is my sample but theirs all turned out super cute!!
I found some already made memory book pages on myteacherpages.com by a lady named Joanne Griffin. They were really cute so I just downloaded them and changed some of the information to fit my needs:)
These are just pictures of the front cover and first page.
There were six pages in all in my book.
